Informed Consent

Dear Sir or Madam:

You are invited to participate in an online survey on the effectiveness of online learning activities. This research project is being conducted by Melissa Jakubec, an Instructional Designer at Thompson Rivers University. It should take approximately 15 to 20 minutes to complete.

PARTICIPATION
Your participation in this survey is voluntary. You may refuse to take part in the research or exit the survey at any time without penalty. You are free to decline to answer any particular question you do not wish to answer for any reason.

PURPOSE
You will receive no direct benefits from participating in this research study. However, your responses will help me learn more about the characteristics of learning activities perceived to be effective by instructional designers and educators. The purpose of this project is to ascertain which learning activities instructional designers and educators perceive to be most effective in online environments. Another purpose is to gather instructional designer and educators’ views of which characteristics contribute most to the effectiveness of a learning activity. Data will be compiled by the researcher and may be used in presentations and publications.

RISKS
There are no foreseeable risks involved in participating in this study other than those encountered in day-to-day life.

CONFIDENTIALITY
Your survey answer data will be stored in the Fluid Surveys database on a server at Thompson Rivers University in a password protected electronic format. Fluid Surveys does not collect identifying information such as your name, email address, or IP address. Therefore, your responses will remain anonymous. No one will be able to identify you or your answers, and no one will know whether or not you participated in the study. At the end of the survey you will be asked if you are interested in participating in an additional interview [by phone, in person, or email]. If you choose to provide contact information such as your phone number or email address, your survey responses may no longer be anonymous to the researcher. However, no names or identifying information would be included in any publications or presentations based on these data, and your responses to this survey will remain confidential.
